Handling of Personal Information

This is in reference to the handling, etc. of the personal information disclosed to
Pasona Inc. (hereinafter referred to as “Pasona ”) by customers for their use of
the “Pasona Aptitude Test” (hereinafter referred to as the“Diagnosis Tool”),
the tool developed to find a suitable job for customers, during the process of
the Employment Assistance Service (hereinafter referred to as the “Service”)
provided by Pasona. The customers are requested to read and agree to the items
listed below before they use the Diagnosis Tool.

1. Personal Information and Purpose of Use of Such

1) The personal information refers to the name (its pronunciation in kana), date of
  birth, sex, E-mail address and telephone number that are input by the customers using
  the“Personal Data Initial Registration Form” of the Diagnosis Tool.
2) The personal information registered as above will be used for the purposes ofi
  analysis of the responses and announcement and management of the diagnosis results.
  Pasona may possibly distribute information related to the Service using
  the personal information. In addition, Pasona will contact the customers
  by E-mail or telephone, etc. to notify them of their diagnosis results.
  The personal information registered by the customers and their diagnosis results
  will be disposed of when six (6) months have elapsed from the date of diagnosis.
  Please be advised accordingly in advance.

2. Outsourcing of Handling of Personal Information

  Management and operation of the Diagnosis Tool is entrusted to “Nippon EAP Center
  Co.,ltd” The parties to be entrusted with the handling of the personal information
  are selected through rigid examination and approval procedures performed by
  Pasona and they are entrusted with the work under a nondisclosure agreement
  entered into with them as well as after the confirmation of their guidelines for
  personal information.

3. Provision of Personal Information to Third Party

  The personal information registered by the customers will not be disclosed to any
  third party without the consent of the customers.

4. Voluntariness of Registration of Personal Information

  It is left to the customers’ discretion whether to register their own information,
  but the purposes of the use of the Diagnosis Tool may not be fully achieved if the
  necessary information is not registered.

5. Disclosure, etc.

  For a request for the purposes of use, disclosure, addition, suspension of use,
  erasure and suspension of provision to third parties of the personal information,
  customers are requested to contact Pasona office stated below.
  As confirmation of the identify and contents of the request are
  required at the time of making the request, customers are requested
  to have the requisite information ready for submission in the form
  designated by Pasona.
